Why Male Pattern Baldness Happens
It’s 2024 and many of us who have done our own research no longer buy into the “DHT causes hair loss” theory because it’s easily debunkable. That’s not to say it doesn’t play a role in hair loss, of course it does: evidence suggests 5-Alpha Reductase (the ezyme that converts testosterone to DHT) is upregulated in certain tissue under inflammatory conditions, particularly high TNF-a. It’s then that AR mediated gene transcription increases the expression of TGF-beta 1, a protein responsible for the stopping of hair growth, apoptosis (cell death) of kertinocytes, the development of scar tissue & hardening of hair follicles that ultimately leads to hair loss. Luckily, this suggests that there are more effective solutions than suppressing DHT/AR transcription and also explains why drugs like finasteride don’t always work.
The true root cause of MPB appears to be scalp tension. The galea aponeurotica is a type of tendon that serves as an attachment point for the frontalis and occipitalis muscles. As you can see in the Norwood scale of male pattern baldness chart below, hair loss only occurs only in regions above the galea. .
This is backed by a 2015 study found that the areas of the galea experiencing the highest tension correspond to the regions where hair loss begins and progresses. The chronic tension in the galea may lead to micro-inflammation in the overlying skin, increasing pro-inflammatory cytokines like TNF-a, which then upregulates 5AR, which then increases DHT, which then increases TGF-beta1 that cause fibrosis around hair follicles. Fibrosis restricts blood supply to the follicles, depriving them of oxygen and nutrients, which causes MORE inflammation and the cycle continues until the hairloss follicles miniaturize and eventually die.
This explains why, over half of men on finasteride do not grow hair and a quarter notice no benefits at all. DHT is not at the root cause, and reducing serum DHT levels through 5AR inhibition has a ton of negative side effects: mentally (cognitive deficits, depression, anxiety, loss of confidence), physically (too many to name), sexually( ED, penis shrinkage, low libido). Since tissues saturate with androgens at fairly low levels, it also seems likely that a lot of finasteride’s effect is driven by directly blocking androgen receptors which causes the TGF-b1 suppression, but no one is talking about this.
